Prior studies concerning the persistence of golimumab (GLM) therapy in Japanese rheumatoid arthritis (RA) cases have been conducted; however, further research is needed to demonstrate its long-term effectiveness in the real-world clinical setting. In Japanese clinical practice, this study investigated the sustained application of GLM therapy in rheumatoid arthritis (RA) patients, encompassing factors impacting its longevity and the influence of pre-existing medications.
A retrospective cohort study, centered on rheumatoid arthritis, was conducted using a Japanese hospital insurance claims database. Identified patients were categorized: those receiving only GLM treatment (naive), those with one prior bDMARD/JAK inhibitor treatment before GLM [switch(1)], and those who had used at least two bDMARDs/JAKs before GLM treatment [switch(2)] . Descriptive statistics were applied in the evaluation of patient characteristics. GLM persistence was evaluated at 1, 3, 5, and 7 years, and its associated factors were determined via Kaplan-Meier survival and Cox regression procedures. The log-rank test facilitated the comparison of treatment differences.
The GLM persistence rate for the naive group was observed to be 588%, 321%, 214%, and 114% at the conclusion of 1, 3, 5, and 7 years, respectively. From an overall perspective, the persistence rates of the naive group were superior to those of the switch groups. Persistence of GLM was observed more frequently in patients 61 to 75 years old who were also using methotrexate (MTX). Women, on average, were less likely to cease treatment than men. A diminished rate of persistence was found among patients with a higher Charlson Comorbidity Index, those initiating GLM treatment at 100mg, and those changing from prior bDMARDs/JAK inhibitor therapies. Infiliximab, as a prior medication, demonstrated the greatest duration of subsequent GLM persistence, setting a benchmark that was significantly surpassed by shorter persistence durations for tocilizumab, sarilumab, and tofacitinib subgroups, respectively (p=0.0001, 0.0025, 0.0041).
The sustained impact of GLM in a real-world setting and factors associated with its persistence are presented in this study. These observations, both recent and long-term, point to the persistent advantage of GLM and other bDMARDs for treating RA in Japan.
